Explanation of the Counseling Information Form

The Counseling Information Form is the initial intake form used by the Small Business Development Centers (SBDC). A new client needs only to complete the form and sign it and date it. All the data captured on this form is strictly for statistical purpose and is held in strict confidence to the extent allowed by law.
