MO PTAC Forms and Documents


The MO PTAC provides commonly used forms and documents online. Some are available in both Microsoft Word format and Adobe's PDF format.

The following forms are currently available:

Initial Client Forms

  • Request for Counseling: Sign up for services online
  • Download forms: [PDF] [Word]
  • Supplemental Information (page 2) [Word]
    This form provides the geographic and demographic information necessary to initiate a counseling session. Potential clients should complete the form with only 3-4 blanks left incomplete. The counselor will assist on any questions the client has. (Signature Required)
  • Agreement of Understanding [PDF] [Word] updated 6/30/09
    This form signifies that the client is interested in assistance in sales and marketing assistance to state and federal government agencies. Counselors use this form as an outline to explain the benefits and services of MO PTAC. By signing the form the client indicates agreement with their responsibilities to provide monthly client status forms with information on bid leads, ordering solicitations and successful bidding on contracts. (Signature Required)

Client Forms for Reporting

  • Monthly Client Status Report (MCSR)
  • Monthly Client Status Report (for St. Louis clients)
  • Monthly Client Status Report (for Rolla clients)
    This form is a required monthly report of a businesses efforts and success in marketing to governmental agencies. MO PTAC must validate the effectiveness of their services to the granting agency. By showing that clients are ordering bid solicitations, ordering bid packages, bidding on solicitations and occasionally receiving government contracts, MO PTAC will remain available to small business owners. (Signature required on a monthly basis.)

Buying Activities List

  • Federal, State and Local Government Buying Activities in Missouri [Word] - updated 1/18/08

The list of Missouri Buying Activities is a listing of major federal, state, local government and Department of Defense prime contractor buying offices.

You as a contractor should consider reviewing this list and identifying those organizations with whom you would like to do business. For Department of Defense buying offices, you must be registered in the Central Contractor Registration (CCR) system. Other federal buying activities may select their list of companies for bidding purposes from the Small Business Administration's PRO-Net system. The state of Missouri also has a registration system in which you must be registered. Other buying offices may have other registration processes in which you must be registered. The Missouri Procurement Technical Assistance Centers (MO PTAC) can assist you with these registrations.

Registration is just the first step. You may improve your chances of receiving a government contract by marketing directly to those buying offices. Visitation to the person's office is the preferred method. It is understood that there are many demands for your time, so telephone calls, mailing or e-mailing company information is also suggested.

This is only a limited listing of government buying offices that may be interested in your product or service. Others include: school systems; police departments; water districts; sheriff departments; county commissioners; county road districts; public works departments; and others to name a few.
